Out of Area Placement

Description

Provision of residential care to a service user to be nearer their family. SU was placed on 26th August 2025, however this was not picked up until supplier setup. The placement was based on the preference of the service user and family. Cost is £1035 per week. This agreement is for an indefinite period and will continue until it is ended by either the council or the care home. It can be ended by providing written notice, as detailed in the contract. The contract will also set out the circumstances in which the care home may provide notice to end the agreement.
