Delivery of a community stroke navigator service to support reintegration into the community, volunteering, return to work and support stroke survivors, their families and carers to navigate health and social services in Haringey and Islington - includes six month reviews. Communication service - to provide support and rehabilitation for people who suffer from speech and language problems as a result of a stroke, working in conjunction with speech and language therapists.
