It will provide a single-entry point for residents seeking lifestyle and behavioural support, while also addressing wider determinants of health such as emotional wellbeing. The service will adopt a household approach, working in partnership with local neighbourhood models and community assets to deliver flexible, person-centred interventions. Delivery will include community-based and digital options, ensuring accessibility and alignment with Bolton's vision to improve health and wellbeing for all age groups.
