Provision of Rapid Winter Productivity Support (Phase 2)

Description

Phase 2 of the Rapid Winter Productivity Support provided by IMPOWER. The predominant focus of the work will be on delivering a range of interventions on all Health and Care of Older People (HCOP) wards, with a focus on reducing the length of stay, whilst maintaining good discharge outcomes. We will do this by focusing on changing behaviours on wards and supporting the optimisation of the virtual frailty ward (VFW).
