Home from Hospital Service

Description

The provision of the Home from Hospital Service is intended for Haringey residents aged 18 and over, supporting them to return home safely from hospital through effective discharge. The service aims to: • Reduce the number of re-admissions within 28 days for residents who have accessed the Home from Hospital Service (excluding those with scheduled appointments). • Ensure supported discharge from emergency departments and medical assessment wards to avoid unnecessary admissions. • Increase the number of people who are able to remain sustained in their communities following a hospital admission.
