Extension of the Frailty ACE service delivered under the BrisDoc Severnside contract to support admission avoidance. The F-ACE hub is co-located with the Severnside Weekday Professional Line to facilitate real time awareness of F-ACE capacity, direct handover, and minimised duplication. The F-ACE extension will now align with the BrisDoc Severnside contract expiry date. This modification adds an additional £4,227,676.00 to the lifetime value of the contract.

Jisc Services Limited has entered into a contract with ProQuest LLC to give Jisc member institutions the opportunity to subscribe to the ProQuest Film, Music & Performing Arts Collection at preferential rates. The film collection contains current publications related to film scholarship alongside detailed and expansive filmographies. The music & performing arts collection combines cross-searchable, seminal bibliographies in music and performing arts, with full-text from hundreds of journals. This contract is for use by Jisc Members only and not for Jisc's own use.

This service provides routine elective secondary care, outpatient, diagnostic and treatment services for eligible patients under patient choice arrangements. The service supports access to commissioned specialties including • General Surgery including endoscopy and upper GI procedures • Orthopaedics/Musculoskeletal including joints and pain management (in line with local pathways relating to the management of these conditions). Small volumes of overlapping Spinal treatment are also included where ICB is commissioner. • Ophthalmology • Oral surgery and Maxillofacial • Plastics (dermatology) • Urology The Provider may provide the following services through the contract duration without the requirement of a contract variation: • Ear, Nose and Throat • Gynaecology . The provider is required to deliver the full patient pathway where appropriate, including referral management, triage, diagnostics, consultation, pre-treatment assessment, treatment, discharge and follow-up within the commissioned scope. The service is delivered by Somerset Surgical Services Ltd from Weston General Hospital, Grange Road, Uphill, Weston-Super-Mare, Somerset, BS23 4TQ, which is included as an agreed additional premises for the purposes of this Contract. Any delivery of the service from further or alternative premises will require the prior written approval of the Commissioner.

The provider is required to deliver the full patient pathway where appropriate, including referral management, triage, diagnostics, consultation, pre-treatment assessment, treatment, discharge and follow-up within the commissioned scope. The service is delivered by Nuffield Health from Nuffield Health Bristol Hospital, which is included as an agreed additional premises, with any further premises requiring prior Commissioner approval.

The Provider is commissioned for following services • Orthopedics / Musculoskeletal including joints and pain management (in line with local pathways relating to the management of these conditions) The services will include, where appropriate • Pre-treatment e.g. referral processes; triage/clinical assessment; diagnostics; consultation; pre-treatment assessment and / or work up. • Treatment e.g. outpatient/ambulatory/inpatient treatment • Discharge e.g. expected physical, self-care, psychological capabilities prior to discharge. • Follow up e.g. specialist support post discharge, referrals to other general or specialist services such as GP or District Nurse, self-care requirements for a period up to and including 14 days post discharge service supports access to commissioned specialties including Orthopaedics, Spinal Services (where ICB is Commissioner), General Surgery. The provider is required to deliver the full patient pathway where appropriate, including referral management, triage, diagnostics, consultation, pre-treatment assessment, treatment, discharge and follow-up within the commissioned scope. The service is delivered by Spire Healthcare Limited from Spire Bristol Hospital, Redland Hill, Durdham Down, Bristol, BS6 6UT, which is included as an agreed additional premises, with any further premises requiring prior Commissioner approval.

This service provides routine elective secondary care, outpatient, diagnostic and treatment services for eligible patients under patient choice arrangements. The service supports access to commissioned specialties including oral surgery, ENT, general surgery, gynaecology, orthopaedics/musculoskeletal services, ophthalmology, urology, gastroenterology/endoscopy and agreed diagnostic procedures such as X-ray, MRI and ultrasound. The provider is required to deliver the full patient pathway where appropriate, including referral management, triage, diagnostics, consultation, pre-treatment assessment, treatment, discharge and follow-up within the commissioned scope. The service is delivered from Practice Plus Group Hospital Emersons Green, with Devizes Surgical Centre included as an agreed additional premises, and any further premises requiring prior Commissioner approval
